一、填寫「水上運動會參賽前運動風險問卷」

請各參賽單位將交由各參賽人員填寫再進行報名，幫助判斷您的身體狀況是否適合參賽，或是否需要醫師或其他專業人員的協助。
參加人員應遵照醫囑衡量個人體能狀況，凡患有心臟病、癲癇病、皮膚病及其他不適於游泳之疾病者，請勿報名參加比賽。

參加個人單項及四人4 x 50 公尺接力、智慧鐵人兩項、趣味競賽、十人10 x 50公尺大隊接力，請各系負責人(體育幹部)至報名系統報名

2026 National Taiwan Normal University Aquatic Game: Competition Information

A. fill in form

Health Risk Assessment: All participating units are required to ensure that every competitor completes the "Water Sports Meet Pre-Competition Health Risk Assessment Questionnaire" prior to registration. This assessment is designed to help determine whether your physical condition is suitable for the competition or if professional medical consultation is required.

Physical Condition & Medical Advice: Participants must evaluate their own physical fitness in accordance with professional medical advice. Individuals suffering from heart disease, epilepsy, skin diseases, or any other conditions unsuitable for swimming are strictly prohibited from registering for the competition.

Includes Master’s, In-service Master’s, and Doctoral programs. For individual events,4x50m Medley Relay、Duathlon、Fun Races an 10x50m Freestyle RelayThe department representative (Athletic Coordinator) must register through the Online Registration System. Once completed, please upload the registration form to the "2025/2026 Water Sports Meet Cloud Upload Link.
